Full mouth implants replace an entire arch — or both arches — with fixed teeth anchored to implants rather than resting on your gums. The practical question is almost always how many implants that arch should have.
More implants means more surface area for the bone to fuse to, forces spread more evenly, and shorter cantilevers at the back of the arch where leverage is highest.
The two options side by side
All-on-4
Four implants per arch, $18,000 to $25,000. Angled placement often avoids the need for grafting, which makes fixed teeth achievable for patients with reduced bone.
All-on-6
Six implants per arch, $24,000 to $36,000. Better load distribution and reduced cantilever length, at the cost of requiring more bone and more surgery.
Longevity — Both
Twenty to thirty years or more for either system when well maintained. Longevity is driven more by bone quality, bite force, and hygiene than by implant count alone.
What determines which you can have
Bone height is the primary determining factor. In the upper jaw, we need sufficient bone above the sinus floor; in the lower jaw, sufficient clearance above the inferior alveolar nerve.
A 3D CBCT scan measures both precisely. It is not a judgement call — the scan either shows the height and width required, or it shows what grafting would be needed to create it.
Common questions
Does All-on-6 last longer than All-on-4? +
Both systems commonly reach twenty to thirty years or more. All-on-6 distributes load better and carries a larger safety margin, but a well-planned All-on-4 in suitable bone is an excellent long-term result.
Is the surgery more complex with six implants? +
Somewhat. It takes longer and requires more bone volume, but it is the same category of procedure and is performed under the same IV sedation.
Do I get temporary teeth on the day? +
Yes. A provisional prosthesis is fitted the same day in both systems, so in the great majority of cases you leave with fixed teeth in place.
Why is All-on-6 more expensive? +
Two additional implants, additional surgical time, and a prosthesis engineered for six anchor points rather than four.
Will I need bone grafting? +
It depends entirely on your scan. All-on-4's angled placement is specifically designed to avoid grafting where possible; All-on-6 more often requires adequate existing bone.
How often are follow-ups? +
Regular maintenance visits, typically every three to six months initially, then settling into a routine schedule once the arch is stable.
Can both jaws be restored? +
Yes. Upper and lower arches can both be restored, either together or staged, depending on your clinical situation and preference.
