What Is All-on-4?
A revolutionary approach to full-mouth restoration
All-on-4 uses four titanium implants positioned at specific angles to support a full arch of teeth (either upper or lower). Two implants are placed at the front where bone is typically denser. Two implants are angled at the back, utilizing existing bone and avoiding sinus or nerve structures. This angulation, called tilting, allows us to avoid complex bone grafting in many cases.
The four implants support a custom-designed bridge. The bridge is typically temporary on day one (made of acrylic) and replaced with a permanent prosthesis (acrylic or ceramic hybrid) after several months of healing. Some patients keep the temporary bridge permanently—it's durable, easy to maintain, and highly functional.
All-on-4 solves multiple problems: it eliminates dentures (no adhesive, no removal, no slipping), prevents continued bone loss, and restores natural eating and speaking. Patients report confidence, comfort, and quality of life improvements they didn't anticipate.

What to expect during treatment
Before surgery, we perform a 3D CBCT scan to assess bone volume, density, and anatomy. Computer-guided surgery software uses this data to plan implant positions precisely. Dr. Singh reviews the plan with you and makes any necessary adjustments based on your anatomy.
Surgery takes 2-4 hours depending on complexity. Dr. Singh extracts any remaining teeth, places four implants, and typically attaches a temporary acrylic bridge the same day. You leave with teeth—a significant psychological benefit for patients who've worn dentures.
The temporary bridge is for function and esthetics. Over the next 3-6 months, osseointegration occurs. You follow a soft diet regime and post-operative instructions. Once fully healed, a permanent prosthesis is fabricated in our in-house lab and attached, completing the process.

Even if bone resorption has occurred, All-on-4's angled positioning often eliminates the need for grafting. However, severe bone loss may require grafting before implant placement. Yes. Dr. Singh typically attaches a temporary prosthesis the day of surgery. You leave with teeth immediately. The temporary bridge is functional and esthetic, though you follow a soft diet initially. After 3-6 months, we replace it with a more permanent prosthesis.

Implants themselves can last 20-30+ years with proper care. The prosthetic teeth eventually wear and may need repair or replacement—typically after 10-15 years depending on material and maintenance. The implants underneath continue functioning. Many patients have the same implants with several prosthesis replacements over time.
Some patients require bone grafting before All-on-4. We use predictable grafting techniques. Grafts integrate over 4-6 months, then implants are placed. This extends timeline but allows patients with bone loss to become candidates. Discuss grafting needs during your consultation.
After initial healing (soft diet for 1-2 weeks), you can eat most foods normally. You brush and floss your prosthesis like natural teeth. You cannot remove it for cleaning (it's fixed). We recommend professional cleanings every 3-4 months to monitor the health of supporting bone and gums. Avoid hard candies and don't use your teeth as tools.
We assess your medical history carefully. Conditions like diabetes, high blood pressure, or heart disease don't automatically disqualify you but may require additional precautions.
