Two Types of Implant Failure
Early failure occurs before the implant has fully integrated with the bone — typically within the first few months after placement. The implant fails to achieve osseointegration and becomes mobile. Causes include infection at the surgical site, insufficient bone quality or volume, premature loading of the implant, surgical trauma to the bone, and patient factors such as uncontrolled diabetes or smoking.
Late failure occurs after the implant has been functioning — sometimes for years. The most common cause is peri-implantitis, a bacterial infection that progressively destroys the bone around an integrated implant. Other contributing factors include mechanical overload from bruxism or a poorly designed restoration, fracture of the implant or abutment, and systemic health changes that compromise bone integrity.

Early intervention is critical for successful outcomes
Signs that an implant may be failing include pain or discomfort at the implant site, swelling or redness of the surrounding gum tissue, increased probing depths around the implant, mobility of the implant or restoration, and visible bone loss on imaging. Any of these symptoms warrant prompt evaluation.
Early intervention — when the problem is still at the peri-implant mucositis stage — has a far higher success rate than treatment after significant bone loss has occurred. Don't wait if you suspect a problem.
Yes. If an implant does fail, the situation is manageable. The implant is removed, the site is allowed to heal (and grafted if necessary), and a new implant can typically be placed once the underlying cause has been addressed.
